CalAIM PROGRAMS ARE LIVE

New Life Dream Center is proud to announce the launch of our CalAIM (California Advancing and Innovating Medi-Cal) program, in partnership with Contra Costa Health Plan (CCHP).

CalAIM is a statewide initiative designed to improve health outcomes for Medi-Cal recipients by addressing the social drivers of health and providing whole-person, integrated care.

Currently Live

We are now live with Enhanced Care Management (ECM) and the Housing Triad, which includes:

• Housing Transition Navigation Services

• Housing Deposits

• Housing Tenancy and Sustaining Services

Coming Soon

Additional CalAIM Community Supports will be available soon, including:

• Sobering Centers

• Recuperative Care (Medical Respite)

• Short-Term Post-Hospitalization Housing
