CalAIM Services – Go Live Date of September 1st 2025
New Life Dream Center is proud to announce the upcoming launch of our CalAIM (California Advancing and Innovating Medi-Cal) program, in partnership with Contra Costa Health Plan (CCHP).

CalAIM is a statewide initiative designed to improve health outcomes for Medi-Cal recipients by addressing the social drivers of health and providing whole-person, integrated care. Through this initiative, we will offer Enhanced Care Management (ECM) and seven Community Supports (CS) services to help individuals stabilize their lives and thrive.

Our CalAIM Community Supports will include:

• Housing Transition Navigation Services

• Housing Deposits

• Housing Tenancy and Sustaining Services

• Short-Term Post-Hospitalization Housing

• Recuperative Care (Medical Respite)

• Day Habilitation Programs

• Sobering Centers
