The New Life Dream Center is proud to provide CalAIM (California Advancing and Innovating Medi-Cal) services in partnership with Contra Costa Health Plan (CCHP). Since launching the program, we have been privileged to serve members throughout Contra Costa County by providing Enhanced Care Management (ECM) and Community Supports that connect individuals with healthcare, housing, behavioral health services, transportation, and other essential resources. Our mission is to walk alongside each member, helping them achieve greater health, stability, and independence.
